Why “a vaccine is waiting for you” outperformed expert intuition
A megastudy randomized 689,693 Walmart pharmacy patients across 22 text-reminder conditions and a business-as-usual control. Tested reminders increased influenza vaccination by an average of 2.0 percentage points; the strongest approach used repeated messages saying a shot was waiting for the patient.
What transition is actually failing?
Low vaccine uptake can reflect many barriers: awareness, access, trust, intention, timing, or practical friction. A generic information campaign cannot reveal which transition is failing.
What the study tested
The study compared many behaviorally informed reminder variants within one large field setting, allowing message approaches to be evaluated against the same operational outcome.
How the framework reads the result
The winning message may have increased expected relevance, made availability concrete, and shifted the operative frame from abstract recommendation to an already-prepared next step. Repetition may also have improved retrieval at a convenient moment.
